Mentalization Based Treatment (MBT) is an effective, evidence based treatment for Borderline Personality Disorder (BPD) that isn’t widely practiced in the United States.

And it’s being adapted, successfully, to treat a myriad of other mental health conditions as well, including eating disorders, PTSD, substance use disorders, and other personality disorders.

In one study comparing MBT versus treatment as usual for folks with BPD, those treated with MBT showed the following improvements that were sustained over five years after treatment had concluded:

  • fewer suicide attempts

  • fewer mental health services

  • fewer hospitalzations

  • fewer psychiatric medications

  • fewer emergency room visits

  • greater participation in school and employment

MBT theory views the core problem at the heart of BPD as a difficulty with consistently and accurately mentalizing. “Mentalizing” is a complicated-sounding word that refers to the basic, essentially human process of trying to understand ourselves and others in the context of mental states, such as wishes, beliefs, hopes, fears, and desires. Unlike other forms of therapy, which might prioritize the learning of coping skills, or the resolution of internal conflicts, MBT stays focused on supporting a person’s ability to mentalize, and to help them to sustain this ability in the face of distressing emotions that can arise in close relationships.

How is MBT different?

MBT is a structured treatment that typically involves one weekly individual therapy session and one weekly group therapy session. You may find MBT more structured than traditional psychodynamic, exploratory therapies. You may also find MBT less structured than some manualized, behavioral therapies.

In the beginning of your MBT treatment, you can expect to receive some psychoeducation on mentalization and MBT. Together, you and your therapist will develop a written formulation of the problems you are experiencing, as best you understand them. You can expect some support in clarifying these problems from your therapist. This will help keep your therapy focused on the specific difficulties that you are having, so that you can make meaningful progress towards your goals, which you will also define in the beginning of your therapy.

As therapy progresses, both group and individual therapy sessions will usually involve you and your therapist turning your shared attention towards recent difficulties you experience each week. This could be a conflict with a loved one or co-worker, a binge eating episode, or even procrastinating on an important deadline — it all depends on where and when you as an individual are experiencing problems maintaining mentalizing and your individual goals and focus for your treatment.

Do I have to attend both weekly individual and group therapy?

This depends on your treatment needs and expectations. MBT is a manualized treatment protocol that involves both once weekly individual and group therapy. However, it is also possible to participate in weekly individual sessions that are MBT-informed without participating in groups. How much do sessions cost?

Individual sessions are $250 per 50 minute session

Group sessions are $500 a month for weekly 75 minute sessions
